How a nurse-led team can improve heart health

Shirley McGaffin looks at how nurse leadership can improve cardiovascular outcomes

The ‘Our Hearts Our Minds’ (OHOM) programme was set up in 2019 by Dr. Susan Connolly, Consultant Cardiologist at the Western Health and Social Care Trust, and run by a nurse-led team as part of the Department of Health’s Transformation agenda. The 12-week programme was constructed to support cardiovascular patients in making healthier lifestyle choices, ensuring they were on appropriate medication for their blood pressure and cholesterol, as well as promoting psychological well-being.

OHOM for Cardiovascular Health is delivered by a multidisciplinary team (cardiovascular nurse specialist, dietician, physiotherapist/exercise activity specialist, clinical psychologist) supervised by a Consultant Cardiologist. The programme engenders healthy lifestyle change, as well as managing medical risk factors for CVD to target, optimise cardioprotective medications and improve psychological health in those with established cardiovascular disease or those at high multifactorial risk.
